Rates:
$175 for 50-minute Intake
$135 per 50-minute session
$300 per hour for depositions or court appearances. This rate charged includes all time traveled and any additional travel expenses e.g. bridge toll,ferry tickets etc.
$100 per hour for all requested reports.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Is my provider in network?
- What is my coverage for out of network providers?
I am a PCP for Premera Blue Cross, Regence Blue Shield of WA, and Lifewise.
Payment
Cash, check or payment through VENMO. Payment is preferred at the time of the visit. I do bill insurance companies, but arrangements need to be made at your first visit for payment. Even if I do bill your insurance expect to pay for the first visit ($175) at the time of the visit. I do not accept credit, debit, or cards given to charge your pre-tax medical savings. You may use credit or debit cards if your are paying through VENMO. I do not know if you can use cards charging your pre-tax medical savings account, you will need to check with your companies HR department.
I do send out monthly statements to all clients showing the status of the account.
Distance sessions must be paid through VENMO* at the time of session. You will need to download VENMO on your devise. Please be aware VENMO is not HIPPA compliant. I do have an account designated as private and you will need to mark your payments as private if you choose to use this service. If you prefer not to use VENMO you may pay by check.
If you have a balance exceeding $500 past due, and have not contacted me to make payment arrangements, I do turn unpaid accounts over to collection.

Cancellation Policy
If you do not show up for your scheduled appointment or are unable to be reached at the time of your distance appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a missed session fee of $70, due at your next visit. Insurance does not pay for missed appointments.
